Member ID: (or Contract ID) If you have an existing account with MedVantx and know your Member ID or if your prescription information has been transferred to MedVantx, please enter the requested information in the designated boxes. This number may be found on your prescription card or prescription documents received from MedVantx. If you are not familiar with your Member ID, please call our Customer Service Department for assistance at 866-744-0621.

Person Code: (Not required for all accounts) Please enter your two-digit person code. It is typically "01" (or "00") for the primary card holder, "02" (or "01") for the spouse, "03" (or "02") for the first dependent, "04" for the second dependent, etc.

Date of Birth: The Date of Birth must correspond to the Member ID being entered.

Once you have successfully entered your Member ID, Person Code and Date of Birth, you will be prompted to establish a Username and Password.

If you are new to MedVantx, in order to establish your account, you will need to provide your name, address and phone number. We will also ask you to provide your allergy and health information.
